This Complaint Form must be completed in order for your complaint to proceed.

It is strongly advised that before completing the form you read the Complaints Procedure and also the Frequently Asked Questions (FAQs).

You can submit your complaint online using the Online Complaint Form below. Alternatively, you can download the Complaint Form in PDF format, complete the PDF form on your computer and print it, or print it out and complete it by hand. The mailing address is provided at the foot of the PDF form.

In certain circumstances you can lodge a complaint on someone else’s behalf; if, for example, the complainant has a language/writing difficulty. If you are complaining on behalf of someone else, ignore this form and instead fill out the Third Party Complaint Form. If you are making a complaint on your own behalf, proceed to fill out this form.

Signature and date: In order for your complaint to proceed, you must sign this form and also our Waiver and Consent Form. Signing this form carries an obligation to be completely truthful about all aspects of this matter. Signing the Waiver and Consent Form prohibits you from taking any action against the Media Council or its individual members. If you fill out this form online, you will subsequently be asked to sign this form and the Waiver and Consent Form once they have been received and printed out by our Chief Executive Officer.

If you are making the complaint more than 60 days after the initial publication/posting or more than 30 days after broadcast, explain the delay, and give reasons why yours is a special case for which the time limit should be waived.

While we will usually publish the names of the parties involved, you may request that your name is not included in any published decision if you would prefer to remain anonymous. State the reasons why anonymity is preferred and note that a desire for anonymity does not guarantee it.
