This Third Party Complaint Form must be completed in order for your complaint to proceed.

  • In certain circumstances you can lodge a complaint on someone else’s behalf; if, for example, the complainant has a language/writing difficulty.

  • In order to complain on someone else’s behalf, you must have their signed consent.

  • This form is for those who are lodging a complaint on behalf of someone else or on behalf of an organization. If you are complaining on your own behalf, ignore this form and instead fill out the Complaint Form. If you are making a complaint on someone else’s behalf, proceed to fill out this form.

  • It is strongly advised that before completing the form you read the Complaints Procedure and also the Frequently Asked Questions (FAQs).

  • You can submit your complaint online using the Online Third Party Complaint Form below. Alternatively, you can download the Third Party Complaint Form in PDF format, complete the PDF form on your computer and print it, or print it out and complete it by hand. The mailing address is provided at the foot of the PDF form.

  • All fields marked with an asterisk (*) are mandatory.


Signature and date:
 In order for your complaint to proceed, you must sign this form and also our Waiver and Consent Form. Signing this form carries an obligation to be completely truthful about all aspects of this matter. Signing the Waiver and Consent Form prohibits you from taking any action against the Media Council or its individual members. If you fill out this form online, you will subsequently be asked to sign this form and the Waiver and Consent Form once they have been received and printed out by our Executive Officer.

When you have completed the whole form, click on SUBMIT THIS FORM below to send your complaint to the Media Council of Bermuda. Our Executive Officer will contact you to continue with the complaint process.
